How our questions are made and checked
At a glance
- Original questions. Written for KeyBrainer for the U.S. RN licensure exam. They are not NCLEX® questions and are not copied from any exam or question bank.
- Written with AI, checked by AI reviewers. We say so openly, on every page.
- Every option explained. Each question has a rationale for the correct answer and for every wrong option.
- Not signed off by a licensed clinician. No licensed nurse or other clinician has reviewed each question. AI can make mistakes.
- Corrections are free for ebook buyers. Corrected editions are available to re-download for 12 months.
1. What the content is
KeyBrainer questions are original practice questions in the style of the U.S. RN licensure exam. They follow the topics of the published NCLEX-RN® test plan and general nursing knowledge. According to the owner, no exam, textbook or commercial question bank was copied. We cannot prove that something was not copied, so if you ever see a question that looks like someone else's, tell us and we will investigate and remove it.
2. How the questions are made
- The questions and rationales are written with artificial intelligence (AI) under written rules: new wording only, one correct answer, a rationale for the correct answer and for each wrong option, and U.S. practice as the baseline.
- The original set was generated this way from NCLEX-RN test-plan topics. Newer questions were written in separate batches by AI authors.
3. How they are checked
- AI review against references. Questions and rationales are checked by AI reviewers against reliable, widely used references with U.S. standards as the baseline, for example the NCLEX-RN® test plan, CDC guidance and FDA drug labeling.
- Errors are corrected, and drafts that fail are dropped. Questions the AI review flagged were rewritten and re-checked. Of the newest batch of drafts, 16 failed the AI review and were thrown away. Repeated questions and quarantined items were removed.
- U.S. units. Temperatures show °F next to °C in the items where only °C was given.
- Nothing is published automatically. A set of questions goes live only after the site owner authorizes that release.
- Ebooks match the question set. After building each ebook we compared every question, option, answer letter and rationale in the PDF with the released question set, using an automated program. Latest check, 13 October 2026: 2,200 of 2,200 questions matched, and no question is split across pages or columns.

4. What we do not have
No licensed nurse, physician or other clinician has reviewed or signed off these questions one by one, and AI can be wrong. We do not claim otherwise. If that changes, this page will say who reviewed what and when, and we will not describe anything as clinician-reviewed before then.
5. How to use the questions well
- Use them for practice and to find gaps. Read the rationale for every option, including the wrong ones.
- When a point matters (drug doses, emergency care, local law), check it against your program, your textbook and current guidelines.
- Practice and policy outside the U.S. can differ. These questions use U.S. standards, because the exam does.
6. Found a mistake?
Please tell us on the contact page: the ebook or practice set, the question number (for example Q123) and what looks wrong. We check it, correct it and publish a corrected edition. Ebook buyers can re-download corrected editions for 12 months after purchase.
